What form do I need if deceased wishes to be cremated?

A pre-dead person should state his or her wishes concerning cremation in writing. If death has already transpired with no written declaration concerning the disposition of bodily remains, then the decedent's heirs are entitled to do with the body what they will (within conformity to local laws.) There is no particular form. Purchase of a cremation contract with a funeral home would leave a good trail. In any event, simply tell the undertaker to cremate.
