Divorce Indigent

I have been told there is a form for persons who are considered indigent and cannot afford to pay a filing fee. I live in Kentucky and I can't find the form I need. I was told it may b KRS4530190. This needs to be presented to the Judge. Can you help me find this form? I appreciate any help. I have typed the divorce papers myself and am trying to do this on my own.

Go to the local circuit or family court clerk's office and find out if there is a "Friend of the Court" office in your county. They should have a form or sample "motion and affidavit of indigency" there.

Otherwise, there may be a form at the county clerk's office.

If you do not have luck with either of those, check the nearest Pro-Bono office.
