Legal Question in Insurance Law in Kentucky

Insurance application altered

I am a small business owner and in '99 received insurance with a reputable company. After an incident, I found out that my insurance would not cover it because my business policy did not carry Products completed operation coverage and this was true after reading the policy, it was plain as day, but after speaking to an attorney he received my actual application for insurance and it was shocking. On my application the question of products/completed operations coverage which I just received a copy of has a block to mark yes or no -BOTH are marked and to add to that the application has several other problems and appears to have been altered. The attorney I presently have was gracious enough to send a letter to the insurance company for me stating the problems and wanted to set up a time to which he could view the original application. One last thing that kind of throws this way out the signature on the application is my name but not my writing after looking at tons of specimens. I didn't say I was looking for an attorney but I want to know my options and this could lead into a consent of representation considering my present attorney is too busy. What are my options??
