Deed Name Change

I have dissolved my LLC this month. I own two farms which were deeded under the LLC and need to put this land back into my personal name. What form do I need to complete to then properly register this land at the courthouse?

There is no "form" that you can use. A lot depends on the LLC organization and dissolution. without knowledge of this, it would be difficult to advise you. Normally, a deed from the LLC to you would be necessary. Your safest bet, and to avoid title problems, is to consult the advice of a local attorney familiar with real estate.
