affidavit statement

i am looking for the proper from in order to have have a sworn statement regarding facts of a possible theft of services, receiving a deposit and not perfoming the job due to a partner who spent the deposit in the account

You will need an affidavit written. I don't think there is any form out there with the particular language you need, as affidavits are only infrequently available in "forms."

The affidavit should be notarized and signed in front of two witnesses.
