Name change

My last name was not changed in the divorce decree...what do I have to do to get it changed back to my maiden name?

In Louisiana, a woman does not "legally" take her husband's last name, so technically you can start using your maiden name without anything else. Unfortunately, you probably changed the name on your SS account and other and various legal documents. In order to change those things to your maiden name, you will need to file a name change Petition.
