Legal Question in Medical Malpractice in Louisiana

If someone makes a doctors note with the wrong address and a bogus nurse signature, what legal action would take place. even if this person has never had anything or any charges on their record.

It depends why the person forged the doctors note and what they gained by doing it. This sounds like a criminal case, not a medical malpractice case to me.
