The correct term is quitclaim deed. If you are using a preprinted form, you must put in the names of the current owners as parties of the first part and the persons who will be in title afterword as parties of the second part. You need to indicate how the parties of the second part will hold title, either as tenants in common, joint tenants with right of survivorship or if a married couple, tenants by the entirety. You also need to input the correct legal description as contained in the current deed of record.

There are a number of other requirements, and you would be well advised to let an experienced real estate attorney assist you so that you don't adversely affect the chain of title.
