The insurer probably has a guideline for how much therapy someone with your injury typically needs. They will be receiving progress reports from your therapist. If you are still in therapy as of their arbitrary "deadline", they may require you to get an independent evaluation, and if that evaluation says that you won't benefit from further treatment, they will notify you that they are suspending payment. If your therapist believes you are still benefitting from therapy at that time, you can petition workers' comp to order the insurer to continue to pay.
