Incorp an existing church society

We want to incorporate an existing religious non-profit church/society. Already existing in NC and CT, want to Inc. in MA, and eventually RI.

Had no problem getting the form and filed in CT.

Can't get right form from MA.

The form you seek is for a non-profit corporation. You should be able to get the correct form from the Secretary of States website.

If all you want is a form to create a non-profit corporation, you can get that on-line from the MA Secretary of State's Office.
