I live in mass and am looking to add my name to my child's birth certificate. I have been awarded full sole custody, his mother is no longer in the picture and moved away

Your question is a little confusing because of the terms being used. If you have full legal custody of your child, you should be on the birth certificate. If not, you may want to request an amended birth certificate. If you somehow have custody of a child that is not yours, you will need to adopt the child. Custody might be awarded to the state, to a guardian, a drandparent, or other person who is not actually the child's parent. In order to establish a child-parent relationship to a child who is not biologically yours, you will probably have to adopt. It sounds like you need a lawyer to sort this out.
