You cannot force a landlord to rescind a lease. You send a certified letter to the Landlord that you are no longer able to live as an independent person and that you are relocating to a facility or place that will comply with your medical needs and include a note from your doctor. Inform him that he is to mitigate his losses by placing the apartment up for rent immediately. You can offer to give him your security deposit as a termination payment. Indicate since your only source of income is SSDI, and as such you cannot afford to pay for two living arrangements and as such it is in his best interest to sign a lease termination agreement and surrender of the premises now.

He cannot attach social security checks and if he persists in trying to collect from you, you can file a bankruptcy, which since you are on SSDI, will not be difficult to get granted.
