Legal Question in Medical Malpractice in Massachusetts

I went to a dentist to have my upper teeth removed and the dentist removed all of my teeth but 2.

He read the wrong chart. He prescribed so many pain killers, I fell and broke my ankle on both sides and then my leg the night of the fall because my dog knocked me over! I I thought it was a good case until they produced a consent form signed by me the day of the surgery to remove all my teeth with the discription listed as extraction # and a bunch of numbers written in order and on 2 lines. The lower being last which means they could add it on.

I do not recall signing anything. I was heavily medicated at the time. Also, the office manager admitted her mistake putting the wrong paperwork in my file in front of two other employees the day of the incident. I do have a piece of paper that had the word 'wait' on it for the lower teeth the office manager wrote, and when they sent me their copy, the word 'wait' wasn't on it! I feel as though they altered one paper, they may have done it again with the consent form, which btw, I did not get my copy.
