Legal Question in Workers Comp in Massachusetts

My company is trying to get me to take Short Term Disab.

On Nov 9, I fell off my bucket truck at work because of a broken step on my truck. I broke my clavicle. A week later I went back on light duty but I was having stomach pains so a week after that I took myself out again and demanded further tests be done. That's when they found I had a Hiatal Hernia. This past Jan 3rd I went back on light duty. However, To this date I have only received 2 checks from Kemper Insurance. Both for 300.00 each. That's it. Now my company for some reason keeps pressuring me to sign on for short term disability with ''Work n well'' Somehow, I feel it's because my General Manager at work failed to report the accident properly. I know this because I put in a memo to my safety supervisor about the faulty truck that had yet to be fixed and he said he didn't even know I was injured. So now that my GM's job is on the line I feel they are trying to get me to take the STD program to save their own butts. Remember, on the the application for this program it states clearly that if the accident happened on the job then please contact your HR representative.

What should I do????
