Minor Child for guardianship:sample doc to appoint guardian

Please show me a sample document I can use to designate someone as (temporary?) guardian to my son who lives in Las Vegas; I live in Texas now. His father, who had custody, died recently. He did not inherit any assets. He needs the guardian for his SS benefits, obtaining health insurance and passport soonest for his trip to Europe as part of his High School Band, etc.. Thank you very much for your prompt response.

Guardianship out of state

I am afraid there is no form for a temorary guardianship in Texas. If teh divorce and child custody orders are from Texas, you might try a specific power of attorney to give someone power of attorney over teh child. If the child custody order is from another state, the laws of the child's residence state will control. You still might try the power of attorney. I am sorry, but I do not provide "forms". I will provide a completed power of attorney wit hisntructions for proper execution and dleivery for a fee.

I think that you will be looking for an authorization rather than a guardianship. Guardianships are court ordered relationships and they are quite expensive and time consuming to establish and maintain. If your parental rights have not been terminated by a court, then I would think that you should obtain a copy of his birth certificate, a copy of the father's death certificate and have an authorization. I would provide a power of attorney form for you to send to someone having care of the child, for a fee. This is what lawyers do.
