Health and Safety Disclaimers on DVD

I'm producing an instructional video on the basics of singing. I'd like to find out the proper way to address any health issues (asthma, cardiological problems, neurological problems) at the beginning as a warning to consult a doctor first. How can I word this, and is it bullet-proof?

There are some warnings I would propose you include, but nothing is bullet-proof. Your goal is to minimize your exposure and liability as best you can.
