I need a certified copy of my deceased grandfather's birth certificate in New York State for a dual citizenship application. I also need to amend his mother's name as she used her nickname (not a name change) on it. I have contacted the local supreme court and they have said I need to fill out an index number application, an RJI (which I have but I do not know what to put on it), and a motion. I have no clue where to start, any information/help would be greatly appreciated!
