Who do I have to inform and send the letter to when revoking power of attorney?

You should send the letter to both the person you gave the POA to and to any financial institution where you keep an account. Further You should notify your Stock broker.

If you feel that the person who has your power will further abuse it despite your letter, you should look into placing a legal notice and filing the revocation with the county clerk attaching same to your deed so as to alert a title company to your revocation should the POA beneficiary try to sell your real property.
