Legal Question in Health Care Law in New York

Power of Attorney

My sister is having surgery next week (1/24/07)and she wants me to have Power of attorney. We have chosen to have Durable Effective immediately form filled out to have this in effect as soon as possible. Does this form give me the right to handle her finance, health decisions and her children whereabouts if something happens. (ages 17, 11). My sister is concerned about this and also her pending lawsuit she with a bus company. The judge has stated that a decision must be made by 2/6 on the defendant's side. Do I need any other forms or do I have the right forms for this situation?

Once the forms are filled out where do I take it to validate it?

For the health care, you need a health care proxy effective in NYS. For the other matter, I am not certain what position your sister stands in relative to the lawsuit.
