Changeing a name on a deed

We would like to know what forms do we need to change the name and add a spouse on a Mortgage deed in New York State?

This should simply require filing a new Deed, from the current owner(s) to who will be the new owners. Adding a new name does not make that person responsible for payment of the Mortgage, as that person did not sign the Note. If you need assistance, contact me directly.
