Deed or title to house

I want to change the deed for my house. I want to put my daughter's name on the deed with me and have us be owners by surivorship. Where can i get the form to do this?

Almost anywhere...but why not use an attorney? Do you know how to do it correctly? Do you know where to file it? Do you know what cover sheets to use for filing? Do you know if there is any transfer tax to pay?

Are you aware of the possible gift tax implications? There may be better ways to accomplish your wishes.
