My mother is on my original mortgage but I took her off deed per bank advice. Is she still responsible if my house goes into foreclosure

Yes, since she signed original loan documents, unless she was relieved of further responsibility, in writing (new Note without her signature replacing original Note), by the Bank when you took her name off the Deed, she is still responsible.
