Revocation of Power of Attorney with unrecalled date

I executed two powers of attorney, both naming my daughter as my agent (one in 1992 and one in 2002). I now want to revoke both. Ive seen sample forms of Revocation of POA which state the date of the original POA. I only recall the exact date of the POA in 1992 and I dont have a copy of the 2002 POA. How should I phrase the Revocation?

All Powers of Attorney heretofore executed by me are hereby revoked, effective immediately.

I hereby revoke any and all Powers of Attorney made by me in favor of my daughter XXXXX, this revocation to take effect immediately. It must be signed and notarized. Copies must be sent to your daughter (I suggest certified mail, return receipt requested). If at any time copies were given out (to banks, brokerage firms, etc.) copies of the revocation must also be given to the recipients. I also suggest it be recorded in the county in which you live (check with your county clerk/registrar on how to do it) and where your daughter lives. I also suggest you inform your banks, brokerage firms, etc., where you have accounts, that it has been revoked, and give them copies for their files.
