When both parents have no income and no assets and one parent dies, who is responsible for the hospital and clinic bill? both were in their seventies.

The estate of the dead parent, then the survivor. As a practical matter, if there are no assets, the the survivor does not really have to worry too much. The hospital can get a judgment but they will not be able to collect. There is a caveat for state hospitals, like UNC. They can garnish wages or intercept tax refunds. As your parent is elderly, he or she probably does not work. However, he/she might get a tax refund.
