Legal Question in Family Law in Ohio

I want to change my 4 year olds last name to my last name (Im not married). His biological father signed the birth certificate and i only gave him his last name because we were planning on getting married at the time because of the baby. Now he hasnt seen him in 2 years and is over $6,000 behind in child support. He has a bad court record from burglary, drug charges to domestic violence and he cannot/will not find a job. He says he cares about our son but does nothing to prove it. I dont want my son growing up being known as his fathers son just because his last name. How do i go along with changing his last name in the state of ohio? I know if his father finds out (which i dont know how because I havent talked to him in 8 months and he keeps changing his phone number) he would not allow the name change.
