What determines the exchange of the children if the parent with visitation time has to work and asks the other parent if they choose to accept the children?

The shared parenting plan ought to have spelled out in it (1) each parent's 'parenting time,' (2) a back-up in the event the parent cannot fulfill that commitment--which seems to be your situation--and (3) child care provisions separate and apart from parenting time.
