Downloaded Forms Legal?

I want to download a form to amend a will that I want to change the executor. I need to know that if I have the form notarized that it will not be contested in court. In other words, will the new executor have any legal challenges? What advise do you have for this situation.

You need to draft a codicil to the will. It needs to be executed like a will - Signed on the bottom, and signed in the presence of 2 adult witnesses.
