answering a civil complaint

I need to know a format for answering a civil complaint that was served to me. i am being sued by a credit card company through a law office.

You should never answer a civil complaint yourself on a credit card case. Most times, the proper response is not an answer, but rather, the filing of preliminary objections. While I could be mistaken, I don't believe that anyone is going to give you a form for that. You really need to consult with a consumer attorney.
