Legal Question in Wills and Trusts in Pennsylvania

My father has recently been experiencing confusion (general cognitive functions have been impacted by Parkinson's disease over time). An updated will, giving me Power Of Attorney and naming me estate Executor, had been drafted, but was not yet signed by him. He is highly dependent upon others and needs an empowered person to oversee his medical and financial matters. Under these circumstances, how can I resolve this situation in a legally sound manner? If he is unaware as to what he would be signing, in terms of the Will and Power of Attorney, you would need to have a Guardianship established. There is a decent amount of work in doing that, and the legal fees would change from $2-400 for the documents to probably $1500 or more to establish the guardianship.
