Legal Question in Credit and Debt Law in Texas

i don't understand the questions on a Request for Disclosure I need to fill out due to an old credit card debt lawsuit. can you help?

We handle credit card claims frequently for reasonable fees...and if the claim is enough, you may want to call...answering disclosure is part of the normal lawsuit..but not normal for creditors..
