Father's Custody Rights

I have equal custody of my child and the divorce has been final for approximately two years. My ex-wife's health has decreased since the divorce was final, therefore, I feel she is putting our child's wellbeing in jeopardy. My ex has a brain tumor, this is her fourth. She has had three surgeries and is currently being treated with radiation and chemo. My ex will not acknowledge that her health is a problem and uses her parents as a crutch to assist her with our child. Our custody agreement is not between me and her parents. I fear for my child's safety when she is with her mother, due to her reasoning abilities.

Should I hire an attorney or is there another way of handling this situation? Will this be a hard expensive battle to fight?
