Quit Claim Deed Form - Utah

The property is in Utah. The Grantor lives in Arizona. Page 2 of Utah Quit-Claim deed has wording for Notary in Utah. Can the word Utah be changed to Arizona, or can the paragraph be deleted without rendering the document invalid? Or is there another form I need to use?

The state listed for the notary is the state in which the notary is executing the notarial. In other words, if you sign the document in Az, the notary should change the state to AZ. If you sign in Ut, then use the state of Ut.
