are private medical agencies in violation of the social security law by requiring that patients register their social security number before receiving services?

I would doubt it, but one could always refuse to disclose it.

If one doesn't have a SS number, then what?

It's not the SS law that is at play, but the failure to provide services in an emergency that is going to cause trouble, because if someone is unconscious they can't deny service for lack of a SS number.
