Legal Question in Family Law in Virginia

My divorce was finalized in June of 2009. My ex was court ordered to pay all of the marital credit card debt to me, as well as court costs and reimbursement for monies paid on his loans. I now have to file a Petition for Reinstatement and an Order to Reinstate. I can't find examples or templates online and I have to create both forms myself. How do I do this?

Just file a short motion asking the court to reinstate the case to the active docket of the Court with an Order for the Judge to sign reinstating the case. You might also file a motion for a rule to show cause why the ex should not be held in contempt of Court for failing to meet his court-ordered obligations. You do not necessarily need any fancy forms and the Clerk of Court may help you with all this. Good luck.
