Name on birth certificate

Can a unwed mother choose the last name to put on her child's birth certificate? Such as, can she give her child the last name of a grandparent?

Yes, you should be able to give your child the last name you wish him/her to have. Bear in mind, however, the hospital in which you deliver may not allow you to do so. Practically, hospitals act differently in their handling of birth certificates. Also, bear in mind that if you ever receive any public funds, a condition you might have to meet would be to name the probable father(s) of the child. Then the state child support enforcement agency may bring a paternity action on behalf of your child. Upon a finding of paternity, the judge will likely order a change of the child's last name to that of the father.

And, don't forget the importance of the father's medical records to the child's future health.
