Legal Question in DUI Law in Arizona

I left the state before being sentenced for my dui, what do I do now?
I had received a second drunk driving in the state of Arizona in 2004, and before my final court date I moved out of state. I left because I had a crazy ex-boyfriend who could not be held back by a restraining order, and I truly hope to never run into him for the remainder of my life. I, however, would like to take care of my dui in Arizona so that it won't be haunting me for the rest of my life. I just found out today that my license is no longer eligible in this state or the last one that I lived in. Now what do I do?
