leagal gardian of minor children

i am about two weeks away from starting a two year term of incarsiration. i have 3 minor children?is there a simple document that i can write to give leagal gardian ship to freinds or relitives for the term of my incarsiration? it would be uncontested by reltives to wit mother ,grand mother,ect.ect

Please go to http://www.maricopa.gov/supcrt/ssc, and look for the court's forms, specifically Guardianship only forms.
