Indemnify owner of carpool van

We purchased a 15-passenger van for carpooling our children to school. Three additional families share driving and pay a portion of insurance, maintenance etc. each month. What legal form should we have our drivers sign to hold us harmless, as the owners of the van, in the event of an accident? And what form should we have them sign to agree to pay for or have their insurance handle any expenses related to an automobile accident which may occur while they are driving the van?

You may want to put the van in an LLC.
