can an insurance company change policy numbers every quarter for the same insurance you pay on for the same coverage?

It could but it is doubtful. Policies are usually for 6 months or 1 year, and it is too much trouble to keep changing numbers. There could be valid reasons for having separate identifying numbers/letters so I would need to know your real question in order to know what information you actually want.
