Legal Question in Technology Law in California

I'm need information on which forms need to submited to the courts to file a civil injunction against a online/electronic harrasessment stalkers (I've learned the term is 'gang stalking') I've had for over a year. I only have IP & MAC addresses for a few of the people who are involved so I need to have the ISP records subpoenaed in order to get the true identities of these people. Once I have that then I could file a civil harassment suit against the individuals involved. I'm having some trouble finding the correct forms. Can anyone possibly help me regarding this matter?

First of all, in the practice of law one common reason for not finding a "fill-in-the-blanks" form for some purpose is that none exists, or at least none that is good enough for common acceptance. Law firms sometimes rely on "forms" in the sense you mean; sometimes they modify similar documents they've created in the past; and occasionally, when necessary, they create documents truly from scratch.

One excellent source of forms is the library of fill-in-the-blanks forms created by the California Judicial Council. They can be found on line at www.courts.ca.gov/formsrules.htm. There are many dealing with civil harassment and a few dealing with temporary restraining orders. Some of the "forms" are actually instruction sheets. You might want to look at forms CH-100-INFO; CH-130; SUBP-002; and CD-190 for starters. There are others, so scan the list carefully.

I might add that opening a case, using the forms, and appearing for hearings may be complicated and you should consider getting in-person legal assistance by hiring a lawyer or visiting a legal aid office.
