If the address of the party giving a durable power of attorney changes his/her address, can the durable power of attorney be amended to show the new address, or does a new one need to be created, or is there need for any changes to be made?

The power of attorney instrument can be amended, but does not need to be. It's the person who is the attorney in fact, regardless of his or her address.
