violation of a court order

my daughters father has moved its been 5 months and he has not given his new info address/phone what do i need to file in court

If the current court order specifically requires him to give you that information, you can file a contempt of court action.

Ask for the information in a written letter sent certified mail with return receipt requested. If you still do not get the information, then file a motion for contempt with the court. Attach the court's order and a copy of the letter (with the proof he received it) as exhibits to the motion.

A contempt motion is asks the court to punish someone for violating a court order. Court punishment for contempt is criminal in nature carrying the possibility of a fine and/or jail time.
