Can an employee be disciplined for insubordination

Can an employer discipline an employee, i.e. suspension or letter of reprimand, for failing to submit certification, if the employer notified the employee that failure to submit the completed certification form (given 15 calendar days) may result in disciplinary action?

Both state and federal family leave laws permit the employer to require medical certification as a condition of taking time off for medical leave. Failure to provide such documentation puts your job in jeopardy.
