If someone comes thru the ER and says that they had helped in "setting up/framing" to put an innocent person in jail, does HIPAA protect them?

No. HIPAA protects health information. Also, physician-patient privilege is intended to protect patient privacy, not to facilitate an injustice. :"Setting up" or "framing" a person is itself a crime, not just a past crime but a continuing crime, and you have the absolute right to report this crime to the police.
