Health care proxy and durable power of attorney

My mother wants to name me as Health care proxy and as Durable Power of Attorney.

She lives in California, I live in Texas. Do we need different forms for each State?

Is there any reason to file the forms at the County Clerks office other than to have access to a copy in the event the copy/s in our possession are lost or destroyed?

If either of us moves to a different County within our respective State do we need to have new forms with the new address and re-file the forms at that County Clerks office?

No. No. No. In that order.

If your mother is giving you the power and authority involved, she must use documents that comply with the statutory requirements in California, her place of residence. The place of residence of the person obtaining the power does not matter.
