Legal Question in Intellectual Property in California

I plan to publish a book consisting of several short stories from a copyrighted website.

The stories are from several different individuals, along with my own.

This endeavour is completely non-profit as all proceeds after cost will go to "The Wounded Warrior Project"

What sort of simple release form do I need for each story?

I think you need a "license" or "release" form signed by the copyright holder, who may be the publisher (Web site) or the authors (you'll need to find out). Also, before you start trying to find a suitable blank form, you should ask the copyright holder if he/she/it has a form of their own which they will want to use for this purpose. If not, search the Web for copyright license forms, buy a paperback self-help law book on intellectual property licensing, or see an I-P lawyer.
