Can a insurance company ask for a written statement stating coverage an not except a declaration page as proof of coverage?

Your proof of insurance slip should be enough (the one you have to carry around with you)... otherwise, you have no legal obligation to provide them with anything else (unless subpoenaed after a lawsuit is filed).
