Legal Question in Wills and Trusts in California

Advanced Medical Directive

Can a notary public provide blank Advanced Medical Directive forms for clients? Or is it not advised since we are not supposed to provide legal advice? If it is ok to do so, is there legally correct forms to use or are any forms ok?
