Either go to the attorney who set up the trust, go to a title company and have them do it (they will charge you), or go to your county recorders office and get the appropriate forms (ask the clerk there). It is not terribly complicated to do, just make sure you title it EXACTLY as it appears on the trust and that you pick the correct transfer tax exemption for transferring property which you own to a trust which you own.
