Last Rights

I would like to know which form to use to enclose my last wishes to my children,distribution of assets, obituary, program and any other items to be divided among my children?

you will need a will, a revocable trust if your estate is more than $100,000 [or contains real property with a gross value of more than $20,000] and you wish to avoid probate, a durable power of attorney for asset managment, and an advance health care directive. you can find some of the statutory forms at the state bar website. however, to ensure than your estate plan is prepared properly and implemented correctly you should retain a qualified attorney.
